Planned preventive maintenance (PPR) act forms

We have a planned preventive maintenance (PPR) act form, but it is outdated - written for KTPPN and SHGS-5805-class VSDs. Anyone have act blanks for newer stations, plus walkaround / inspection forms?

Доброго времени суток всем. Тут такой вопрос у нас есть бланк Акта ППР но он устарел так как он делался для КТППН и СУ типа ШГС-5805, может есть в других организациях Бланки Актов под новые СУ. И Акты под обход,осмотр.Заранее спасибо.

U grid = supply voltage. U rated = rated voltage. U tap = SUT tap voltage. F op = operating frequency. ZP / ZSP = as labeled on the form. I rated / I op = rated / operating current. AR / U = auto-reclose / voltage.
